Objective: Lymphoblastic and myeloid leukemias are classified into distinct categories such as L9, L9, L1, AML, CML, APL, and MML, based on the morphology of the predominant cells identified through Romanovsky staining While the clinical significance of these classifications in terms of predicting life expectancy in adults remains uncertain, they hold particular value in the prognosis and treatment response, especially in pediatric cases The objective of this study is to investigate the cytochemical patterns in both acute and chronic myeloid and lymphoid leukemias, to provide a more comprehensive diagnostic framework Material and Methods: The study was conducted following approval from the institutional ethical committee A total of 904 cases with a provisional diagnosis of acute leukemia, as identified through Leishman stained peripheral blood or bone marrow smears, were analyzed The cases were referred to the Oncology Department of Razi Hospital, Rasht, under the Gilan University of Medical Sciences in Iran, between 9099 and 9095 Bone marrow smears were subjected to various cytochemical staining techniques, including AS BI, PAS, AP, AS D, and TBO All specific staining protocols were executed using commercially available kits from Cinagen Biotechnology Co , ensuring standardization across the samples Results: In the initial stages of the study, 445 of the cases were diagnosed based on morphology alone However, when cytochemical staining was incorporated alongside morphological examination, the diagnosis rate increased to 475 Despite the addition of cytochemical staining, 15 of the cases remained undiagnosed These cases were complex and required further investigation using immunocytochemistry techniques, which were not available in the initial setting As a result, these cases were referred to higher medical centers for more advanced diagnostic methods The application of cytochemical staining enhanced the accuracy of diagnosis significantly Cytochemical patterns are known to provide valuable insights into the biochemical nature of the leukemic cells, allowing for more precise subclassification of leukemia types The presence of specific enzymes and substrates within the leukemic cells, such as those targeted by PAS, AP, AS BI, and AS D, provides essential clues to distinguish between different leukemia subtypes This approach not only improves diagnostic certainty but also informs the treatment strategies, as each leukemia subtype may respond differently to various therapeutic protocols This study underscores the significance of combining morphological analysis with cytochemical staining as a reliable method for diagnosing acute leukemias in settings where advanced immunophenotyping techniques may not be readily available While immunocytochemistry offers higher specificity and sensitivity in some cases, its accessibility is limited, especially in regions affected by medical sanctions or lacking advanced laboratory infrastructure Therefore, using a combination of morphology and cytochemical staining
