Urban infrastructure systems are increasingly exposed to long term climatic stressors, including rising temperature extremes, altered precipitation patterns, and increased frequency of compound events While structural health monitoring and digital twin technologies have improved short term condition assessment and performance prediction, their use in long term climate adaptation planning remains limited Most current applications focus on monitoring present conditions rather than supporting adaptive decision making under uncertain future climates This study proposes a climate adaptive planning framework that integrates structural health monitoring data with digital twin models to support long term, adaptive infrastructure management The framework uses monitoring informed digital twins to track performance trends, evaluate climate driven degradation pathways, and test adaptation strategies under multiple climate scenarios Rather than relying solely on static design margins or deterministic projections, the approach emphasizes continuous learning and iterative adjustment as climatic conditions evolve The proposed framework is demonstrated through a case study of representative urban infrastructure systems in Tehran Results show that integrating SHM and digital twins enables earlier detection of climate sensitive degradation, supports comparison of adaptation options, and improves alignment between technical assessment and strategic planning The findings highlight the potential of digital twins to transform climate adaptation from a periodic planning exercise into a dynamic, evidence based process embedded in everyday infrastructure management
