The increasing integration of artificial intelligence, structural health monitoring, and digital twin technologies in urban infrastructure management has introduced new capabilities for monitoring, prediction, and decision support While these technologies promise improved efficiency and risk awareness, they also raise significant ethical, legal, and regulatory challenges that remain underexplored in the civil engineering literature This study examines the non technical risks associated with AI enabled structural health monitoring and digital twins, focusing on accountability, transparency, data governance, and decision authority in public infrastructure contexts Particular attention is given to the risk of opaque decision making, over reliance on automated recommendations, and ambiguity in professional and institutional responsibility These issues are especially critical when digital twins influence safety critical decisions affecting public welfare A structured analytical framework is proposed to identify and categorize ethical and regulatory challenges across the lifecycle of AI enabled digital twin systems, from data acquisition and model updating to decision making and governance The framework is examined through a case study of urban infrastructure management practices in Tehran, illustrating how existing institutional structures interact with emerging digital technologies The findings indicate that without explicit ethical and regulatory safeguards, AI enabled digital twins may undermine accountability and trust, despite their technical sophistication The study argues that responsible deployment of digital twins requires not only technical validation but also governance mechanisms that preserve human oversight, decision traceability, and compliance with public sector accountability principles
