چکیده مقاله
Body shame and ego structure are key concepts in understanding the psychological dynamics of individuals with gender dysphoria Although gender dysphoria has been widely studied from biological and socio cultural perspectives, its psychoanalytic foundations—particularly those rooted in early object relations and ego development—have not been sufficiently explored The aim of this study was to investigate the relationship between body shame and ego cohesion in individuals with gender dysphoria, within a psychoanalytic framework based on internalized object relations and narcissistic vulnerability A sample of 60 individuals diagnosed with gender dysphoria according to DSM 5 criteria, selected from specialized gender identity clinics, participated in the study Quantitative tools included the Body Shame Scale BSS and the Psychodynamic Ego Functioning Questionnaire PEFA Additionally, semi structured clinical interviews were conducted with a subsample of 12 participants, and thematic content analysis was used to identify psychodynamic themes such as psychological defenses, ego fragmentation, and the internalization of the patriarchal, punitive super ego The results indicated a significant inverse relationship between ego cohesion and body shame r = 0 67, p < 0 01 , such that weaker ego cohesion was associated with more intense body shame experiences The qualitative analysis further corroborated this finding, revealing themes such as early narcissistic wounds, disturbed body fantasies, and internalized punitive parental figures Gender and pre or post transition status differences were also observed, influencing ego functioning and shame processing The findings suggest that chronic body shame in gender dysphoria may reflect structural ego deficiencies resulting from dysfunctional object relations and impaired identity formation from childhood Psychoanalytic interventions focusing on strengthening the ego, regulating shame experiences, and integrating fragmented body representations may hold significant therapeutic value
