چکیده مقاله
The Internet of Medical Things IoMT , through continuous monitoring and secure data transfer, plays a critical role in transitional care, particularly for older adults with chronic conditions This study systematically reviews global evidence on the design of integrated IoMT based infrastructures for safe transitional care, highlighting emerging trends, achievements, and challenges Methods: Following PRISMA guidelines, a systematic review was conducted through Scopus, Science Direct, Google Scholar, PubMed, and the Cochrane Central Register of Trials between 2023 and 2025 Keywords included Integrated Infrastructure, Continuous Care, Internet of Medical Things IoMT , Secure Patient Transition, Hospital and Home in English and their Persian equivalents Out of 98 retrieved studies, 28 met the inclusion criteria Original research articles, full text availability Results: The review indicates that integrating IoMT with advanced technologies, remote monitoring via wearables, and machine learning algorithms enables early detection of patient conditions and faster clinical decision making Integrated platforms and digital twins enhance coordination between healthcare teams and families while reducing transition errors Blockchain strengthens data security and transparency, while smartwatches in cardiac patients improve physical activity and long term outcomes such as one year mortality However, system heterogeneity, high initial costs, security concerns, and resistance to technology adoption remain persistent challenges Conclusion: IoMT has the potential to enhance the quality, safety, and continuity of transitional care Nevertheless, successful implementation requires robust infrastructure, standardized frameworks, and rigorous clinical validation, underscoring the need for further large scale studies
