Physical activity (PA) has emerged as a significant factor in enhancing tumor immunogenicity and improving cancer prognosis through various mechanisms involving immune cell mobilization and metabolic regulation. This review article synthesizes current research on how exercise influences the immune landscape in cancer, highlighting its role in promoting the circulation of immune cells, particularly cytotoxic T cells, which are crucial for effective anti-tumor responses. Evidence from preclinical studies demonstrates that regular moderate exercise can modulate the tumor microenvironment, shifting immune cell composition from pro-tumoral to anti-tumoral states, enhancing overall survival rates in various cancer types. For instance, a systematic review indicates that engagement in physical activity is correlated with a decrease in the incidence and recurrence of several cancers, including breast, prostate, and colon cancers, suggesting that PA may help maintain tumors in a 'covert' equilibrium state, delaying or averting clinical diagnosis. The review discusses the interplay between PA and cancer immunology, focusing on how exercise-induced changes in metabolic processes can optimize immune function. Key findings suggest that exercise increases immune cells' infiltration into tumors and enhances their cytotoxic capabilities through mechanisms such as the upregulation of chemokines and the modulation of inflammatory pathways. Specifically, PA has been shown to mobilize leukocytes into circulation, enhancing T-cell abundance and reducing dysfunctional T cells, which is vital for effective anti-tumor immunity. Additionally, the article addresses the variability of exercise effects based on cancer type, exercise intensity, and individual host factors, emphasizing the need for tailored exercise interventions in cancer treatment protocols. For example, studies indicate that the benefits of exercise may vary significantly between cancer types and patient demographics, necessitating personalized exercise regimens to optimize therapeutic outcomes. In conclusion, this review underscores the potential of integrating physical activity into cancer care as a complementary strategy to conventional therapies, aiming to improve patient outcomes by harnessing the immunomodulatory effects of exercise. Future research should focus on elucidating the specific molecular pathways involved and optimizing exercise regimens to maximize therapeutic benefits for cancer patients, as the precise mechanisms through which PA influences immune responses remain an area of active investigation..
