Background: Childhood trauma is a significant risk factor for suicidal thoughts and attempts in patients with Major Depressive Disorder MDD , yet the mechanisms underlying this relationship remain unclear This study aimed to investigate the relationship between childhood trauma and suicidal ideation/attempts in MDD patients, examining the mediating roles of psychological pain and deficits in emotion regulation Methods: This fundamental research employed a descriptive correlational design using path analysis The statistical population included all patients with a diagnosis of Major Depressive Disorder attending psychiatric clinics in Zanjan during ۲۰۲۴ ۲۰۲۵ A total of ۲۱۰ patients ۱۳۵ female, ۷۵ male were selected using convenience sampling Data collection instruments included the Childhood Trauma Questionnaire CTQ , Beck Scale for Suicidal Ideation BSSI , Psychache Scale PS , and Difficulties in Emotion Regulation Scale DERS Descriptive statistics were analyzed using SPSS version ۲۶, and path analysis was conducted using AMOS version ۲۶ Results: Direct effects of childhood trauma on suicidal ideation β = ۰ ۳۸, P < ۰ ۰۰۱ and on suicide attempts β = ۰ ۳۲, P < ۰ ۰۰۱ were significant and positive Childhood trauma significantly predicted psychological pain β = ۰ ۵۱, P < ۰ ۰۰۱ and deficits in emotion regulation β = ۰ ۴۸, P < ۰ ۰۰۱ The indirect effect of childhood trauma on suicidal ideation through psychological pain β = ۰ ۲۱, P < ۰ ۰۰۱ and through deficits in emotion regulation β = ۰ ۱۸, P < ۰ ۰۰۱ was significant Similar indirect effects were found for suicide attempts The model explained ۵۴% of the variance in suicidal ideation and ۴۷% of the variance in suicide attempts Conclusion: Psychological pain and deficits in emotion regulation serve as crucial mediators in the relationship between childhood trauma and suicidal thoughts/attempts in MDD patients Interventions targeting emotion regulation skills and addressing psychological pain may reduce suicide risk in traumatized depressed patients
