Materials and methods: The present study is a mixed study that was conducted in 2019 at Mazandaran University of Medical Sciences face, content, and construct validity was investigated by exploratory factor analysis and confirmatory factor analysis with the participation of 334 managers of hospitals in Mazandaran Province The reliability of the questionnaire was estimated using internal consistency Cronbach's alpha and its stability was estimated through test retest Results: the primary tool was designed with 75 items and 20 factors The construct validity of the questionnaire was extracted through exploratory factor analysis leading to the exclusion of 5 items and 20 factors that explained a total of 59 01% of the total variance The CFA showed a model with suitable fitness for the data The reliability of the tool by Cronbach's alpha and ICC were more than 0 80 and shows the stability of scale Conclusion: Hospital performance evaluation questionnaire using IPOCC approach despite features such as proper reliability and validity; simple scoring and ease of use is appropriate and its use is recommended
