Core Problem: Air pollution and oral health disparities are two pressing urban public health challenges that often affect the same vulnerable populations However, their interrelationship—particularly how environmental exposure may spatially coincide with oral health inequalities—remains poorly understood Both Tehran and Houston, as major industrial and vehicular cities, experience persistent air quality problems and marked socioeconomic variation in access to dental care Understanding their shared and divergent spatial patterns can provide valuable insights for evidence based urban environmental policy Approach: This study employs a comparative geospatial framework using Geographic Information Systems GIS to integrate air quality data PM2 5, NO2 with community level oral health indicators and socioeconomic variables Spatial autocorrelation, hot spot mapping, and multivariate regression are applied to identify statistically significant clusters of pollution and periodontal disease prevalence in Tehran and Houston Cross city comparative metrics are developed to evaluate the influence of urban form, environmental regulation, and access to public dental services Key Findings: Preliminary analysis reveals strong spatial overlap between high pollution zones and elevated periodontal disease rates in both cities In Houston, regulatory interventions and greater service coverage moderate this relationship, while in Tehran, limited preventive infrastructure intensifies it Impact: This study provides the first cross urban spatial comparison linking air pollution and oral health disparities Its findings highlight the need to integrate oral health metrics into environmental justice frameworks and urban health planning across both developed and developing contexts
