Non alcoholic fatty liver disease is often considered as growing health epidemic and multifactorial disease with higher than ۲۵% prevalence that affects up to one third of the adult population, with the prevalence being higher ۷۰% ۹۰% in obese individuals NAFLD is characterized by elevated extrahepatic clinical complications, including: visceral obesity, insulin resistance, dyslipidemia, type ۲ diabetes, hypertension, metabolic syndrome, and cardiovascular diseases Sedentary lifestyles and overfeeding are considered to be the most important factors for NAFLD Currently, there is no specific drug therapy approved for the treatment of NAFLD, Lifestyle interventions in the form of calorie restriction and increasing the physical activity level with an aim to reduce weight remain the cornerstone of NAFLD treatment Exercise is an established and essential component of lifestyle modification for NAFLD disease management Despite numerous studies reporting exercise mediated improvements in NAFLD, there remains a large gap in our knowledge of how to optimize exercise prescriptions and whether the whole body benefits of exercise extend beyond improvements in liver fat Therefore, the aim of this comprehensive review is to investigate the underlings cellular and molecular mechanisms associated with increased physical activity beyond weight loss benefits on the treatment of NAFLD In order to access relevant scientific articles, the following scientific databases were extensively explored: PubMed, SID, Google scholar, Research gate, and science Direct Keywords for the search included: mechanisms of NAFLD, physical activity and metabolic health, aerobic and resistance exercise in NAFLD, inter organs crosstalk and metabolic health, lifestyle management The inclusion criteria for the study were: relevant and reputable studies published in scientific journals A total of ۱۰۰ studies were reviewed, and the results of these studies confirm that, there is now clear consensus that exercise is a ‘polypill’ and power tool for prevention and reversing of NAFLD; we just need to learn how to get patients to take it regularly and seriously
