Methadone Maintenance Treatment MMT remains the gold standard for managing opioid use disorder OUD However, the long term neurocognitive consequences of methadone consumption are a subject of ongoing clinical investigation This comprehensive meta analysis synthesizes the latest evidence from 2020 to 2025 to compare cognitive performance in three critical domains—working memory, attention, and reaction time—between methadone maintained patients and healthy controls Following PRISMA Preferred Reporting Items for Systematic Reviews and Meta Analyses guidelines, a systematic search was conducted across international databases Google Scholar, PubMed, ScienceDirect, PsycINFO and Persian academic repositories SID, Magiran Fifteen primary studies met the stringent inclusion criteria, providing data for over 1,200 participants Standardized mean differences Hedges’ g were calculated using fixed and random effects models where appropriate The results indicate profound and statistically significant cognitive deficits in MMT patients across all domains: working memory g = 1 12, 95% CI 1 35, 0 89 , attention g = 0 98, 95% CI 0 76, 1 20 , and reaction time g = 1 24, 95% CI 1 02, 1 46 These findings suggest that while MMT provides critical behavioral stabilization, it is associated with moderate to large neurocognitive impairments that may impede psychosocial rehabilitation and vocational reintegration The study highlights the urgent need for integrated neurocognitive screening and remediation within addiction treatment frameworks
