Substance use disorders SUD are frequently associated with deep seated psychological conflicts, impaired reflective functioning mentalization , and intense interpersonal guilt, particularly survivor guilt This meta analysis evaluates the efficacy of psychodynamic psychotherapy PDT interventions in addressing these core psychological dimensions A systematic search of Google Scholar, PubMed, and local databases e g , SID, Magiran for studies published between 2020 and 2025 was conducted Seven high quality studies meeting inclusion criteria were analyzed Results indicate that psychodynamic interventions, including Mentalization Based Treatment MBT and Short term Dynamic Psychotherapy STDP , significantly enhance mentalization capacity Hedges’ g = 0 47, 95% CI 0 38, 0 56 and significantly modulate survivor guilt Hedges’ g = 0 38, 95% CI 0 46, 0 30 The findings suggest that psychodynamic approaches offer a robust framework for addressing the underlying personality structures and unconscious guilt that often drive addictive behaviors
