Introduction: Child mental health is a national asset requiring a delivery model that leverages market capacities while upholding social justice Recently, commercialization has introduced structural challenges This study aims to investigate how commercialization creates structural gaps in child mental health service delivery Methods: This systematic review searched credible databases, including PubMed, Web of Science, SID, and Magiran, for articles published between 2015 and 2024 Studies analyzing the interaction between market mechanisms, health system structures, and pediatric consequences were included Results: Commercialization is a multi faceted phenomenon Positively, it attracts investment, fosters innovation, and enhances technical quality Conversely, excessive profit seeking concentrates services in wealthy areas, increases costs, promotes unnecessary care, and excludes disadvantaged groups Structural gaps emerge when the balance between these dimensions is disrupted and government oversight weakens Conclusion: Protecting children's health necessitates a transition from chaotic models to a regulated structure The government must guide the private sector through precise oversight and transparent legislation By strengthening effective dimensions and eliminating inefficiencies, structural gaps can be filled, ensuring justice in access and guaranteeing that children's health is not sacrificed for market interests
