The therapeutic relationship, particularly the dynamics of transference T and countertransference CT , is a cornerstone of psychodynamic theory and practice, yet its specific contribution to symptom reduction in mood disorders remains a subject of ongoing empirical investigation This meta analytic review synthesized recent quantitative findings 2020–2025 to evaluate the efficacy of interventions focused on T and the management of CT in reducing depressive and other mood related symptoms A systematic search of Google Scholar and related databases identified 22 studies meeting the inclusion criteria for two separate meta analyses: one focusing on the effect of Transference Interpretation TI and the other on the effect of Countertransference Management CTM The overall pooled effect size for TI on mood symptoms was found to be small but statistically significant Hedges’ g = 0 13, 95% CI 0 01, 0 25 , suggesting a modest but reliable benefit In contrast, the pooled effect size for CTM was moderate and highly significant Hedges’ g = 0 38, 95% CI 0 25, 0 51 , indicating that the therapist’s ability to recognize and manage their emotional reactions is a more potent factor in treatment outcome for mood disorders Significant heterogeneity was observed in the CTM analysis 𝐼 2 =68 9% , suggesting that moderating variables such as therapist experience and patient personality warrant further investigation The findings underscore the analytical approach that views the therapeutic dyad as a crucial mechanism of change, particularly emphasizing the therapist’s internal work CTM as a key determinant of efficacy in treating mood disorders
