Tabata training, a prominent form of High Intensity Interval Training HIIT characterized by 20 second high intensity bouts followed by 10 second rests, has gained considerable attention among Iranian researchers in recent years This narrative review aimed to comprehensively evaluate the metabolic and functional effects of this protocol in Iranian populations from 2014 to July 2025 Searches were conducted in domestic databases MagIran, SID, Noormags and international databases PubMed, Scopus using relevant keywords, and a total of 24 studies, including randomized controlled trials, quasi experimental, and cross sectional designs, were reviewed Populations included women with polycystic ovary syndrome PCOS , overweight or obese individuals, team and combat athletes, and operational personnel Due to substantial heterogeneity in protocols and statistical reporting, meta analysis was not feasible, and a narrative synthesis approach was applied The reviewed studies demonstrated that Tabata training led to reductions in body mass index 1 5 3 5% , body fat percentage 2 4% , and favorable changes in lipid profiles, including decreased triglycerides, LDL, total cholesterol, and increased HDL In women with PCOS, insulin resistance decreased, and positive changes in adipokines were observed Functionally, VO2max improved by approximately 4 11%, and anaerobic capacity and speed increased, whereas vertical jump performance and flexibility showed inconsistent results Sand based training was associated with improved cardiovascular recovery Overall, Tabata training exhibits promising metabolic and functional effects in Iranian populations and can be recommended as a time efficient intervention for managing PCOS and obesity; however, small sample sizes, short term study designs, and concurrent use of medications or supplements limit the reliability of findings, highlighting the need for long term randomized controlled trials with careful monitoring of diet, sleep, actual exercise intensity, and a focus on sustainable clinical outcomes
