Parkinson’s disease PD presents a complex neurodegenerative challenge, requiring innovative, multidimensional care strategies adaptable across diverse global contexts This study proposes a novel neuroeconomic framework integrating biological, digital, economic, and policy domains to reprogram PD care worldwide The model emphasizes neurotrophic enhancement via Ganoderma lucidum and endurance exercise, artificial intelligence AI driven patient stratification, neuroeconomic return on investment NROI for value based financing, and scalable policy implementation aligned with Universal Health Coverage goals Biological outcomes demonstrate significant increases in Brain Derived Neurotrophic Factor BDNF and Nerve Growth Factor NGF , with the Combination group Ganoderma Exercise exhibiting the highest gains ۶۰% in BDNF and ۵۰% in NGF after ۱۲ weeks p < ۰ ۰۰۱ Clinical improvements parallel biomarker changes; motor function UPDRS III improved by ۱۰ points and cognitive scores MoCA increased by ۵ points in the Combination group p < ۰ ۰۰۱ AI algorithms predicted individual treatment responses with ۸۵% accuracy, enabling dynamic, personalized care adjustments through continuous monitoring of gait, speech, and mobility Economic analysis introduces the NROI metric, capturing cognitive, behavioral, and caregiver burden benefits beyond traditional cost effectiveness Ganoderma monotherapy yielded the highest NROI of ۳ ۲ due to low cost and moderate efficacy, suggesting strong applicability in low and middle income countries LMICs Policy simulations confirm the feasibility of tiered investment approaches: Ganoderma based interventions could reach ۶۰% of PD patients in LMICs within five years, potentially reducing hospitalizations by ۲۵%, while combination therapies enhance quality adjusted survival by ۱۵% in high income countries HICs This integrative framework advances Parkinson’s care by bridging ancient herbal medicine with cutting edge AI and economic modeling, fostering equitable, scalable, and personalized management It highlights the importance of neuroeconomic evaluation in optimizing resource allocation and supports global health goals by embedding innovative financing and digital infrastructure strategies Future research should focus on longitudinal clinical trials, data standardization, and implementation pathways to validate and operationalize this model globally
