Background: Depression and anxiety disorders are among the most prevalent mental health conditions globally, with significant personal and societal burden Individual cognitivebehavioral therapy CBT , family therapy FT , and combined interventions represent three major treatment modalities, yet their comparative efficacy through both direct and indirect evidence has not been comprehensively synthesized in a network metaanalytic framework Methods: A systematic literature search was conducted in PubMed, PsycINFO, Embase, Cochrane Library, and Google Scholar for randomized controlled trials published between 2020 and 2025 A total of 87 studies with 12,847 participants were included in a Bayesian network metaanalysis Effect sizes were calculated as Hedges' g with 95% credible intervals CrI Surface under the cumulative ranking curve SUCRA values were used to rank treatment conditions Results: All three active treatments were significantly superior to control conditions Combined interventions CBT family therapy showed the largest effect on depression g = 0 89, 95% CrI: 0 72 to 1 07 and anxiety g = 0 81, 95% CrI: 0 64 to 0 98 Individual CBT demonstrated moderatetolarge effects on depression g = 0 71, 95% CrI: 0 58 to 0 84 and anxiety g = 0 67, 95% CrI: 0 53 to 0 81 Family therapy showed moderate effects on depression g = 0 52, 95% CrI: 0 38 to 0 67 and anxiety g = 0 48, 95% CrI: 0 34 to 0 63 In direct comparisons, combined interventions were significantly more effective than family therapy alone for depression g = 0 37, 95% CrI: 0 18 to 0 56 and anxiety g = 0 33, 95% CrI: 0 14 to 0 52 , but not significantly different from individual CBT alone SUCRA rankings placed combined interventions first for both depression 82 4% and anxiety 79 1% , followed by individual CBT 61 3% and 58 7% , and family therapy 38 9% and 35 2% Conclusions: Combined interventions integrating CBT with family therapy components yield the highest probability of being the most effective treatment for both depression and anxiety Individual CBT remains a robust firstline treatment, while family therapy alone, though effective, may be enhanced by combination with CBT techniques
